Reablement Services

Caring Connections is dedicated to supporting individuals to regain independence after illness and / or hospital stays. The organisation has a dedicated reablement service that works across Liverpool supporting some for the most vulnerable adults within our region, providing one to one tailored support to reduce the need for long term care provision.

Our charity works closely with local homeless charities to provide one to one support to those experiencing homelessness and with care needs / long term health conditions.

We have a dedicated team of reablement workers managed by Brian Nolan (Reablement Manager) whom provide the service across Liverpool hostels.

We work closely with partners Liverpool YMCA / The Whitechapel Centre / Bronlow Health in the provision of these services.
